Job Description
Position Summary - Compiles and maintains original medical records of patients of the health care delivery system.

Essential Duties - Must be performed in order to carry out the position purpose summarized above: (The following description is a general representation of the key duties and responsibilities of this position. Other duties may be assigned, as required.)
• Analyzes and reviews medical records for completeness and accuracy of documentation according to specified standards.
• Abstracts and codes clinical data such as diseases, operations, procedures, and therapies on medical records according to specified standards.
• Monitors completion of medical records in accordance with time standards.
• Audits incomplete records and prepares reports on delinquencies.
• Confers with doctors, nurses, and other health personnel to assure complete, current, and accurate medical records.
• Compiles, maintains, and reviews logs, reports, and statistical records, and researches records to locate health data as requested.
• Maintains and utilizes a variety of health record indexes and storage and retrieval systems.
• Assists in identification of medical records needed for research using both manual and computer indexes.
• Assists Medical Records Supervisor in special studies or research, as needed.
• Prepares input for computerized medial information systems and for processing vital statistics, insurance, and other forms and documents.
• Operates computer to process, scan, store, and retrieve health information.

• A minimum of one year in a health care environment and customer service support.
• Strong analytical skills and attention to detail.
• Computer proficiency: Microsoft Office Suite, Web maintenance, database management helpful.
• High school diploma or GED.
